Every family needs support after bringing home a baby. The needs are different in each family – from help with feeding and safe sleep to getting information about child care and parenting groups. Family Connects links parents to the individual community resources they need.

The Family Connects Model is an evidence-based model designed to support optimal maternal-child health and advance positive outcomes, while promoting better aligned community care systems.
Connecting Every Family in Colorado to a Healthy Future
Illuminate Colorado is partnering with Family Connects International® to bring home visiting to all families with newborns in Colorado. Currently Family Connects provides home visits in Arapahoe, Boulder, Broomfield, Denver, Eagle, Jefferson and Mesa counties. These counties are expanding, and more counties will be offering Family Connects soon. Illuminate Colorado’s mission is to strengthen families, organizations, and communities to prevent child maltreatment.
