Support R-06 Investments in Family Connects Colorado

Universal Home Visiting Pilot for Colorado Families

The Colorado Department of Early Childhood (CDEC) has requested an ongoing $4,000,000 per year to establish a five-year Family Connects Pilot Program to serve 12,000 births per year across Colorado. Currently in implementation stages in Boulder, Eagle, Jefferson, and Denver counties, this investment would stabilize and expand existing sites to serve additional families by expanding eligibility at current and projected hospitals to residents in surrounding counties. This would enable state-level certification in the model, and fund a Cost Savings Study. Once certified, Family Connects will be poised to scale statewide and improve population health.

What is Family Connects?

Family Connects is an evidence-based home visiting model dedicated to enhancing the well-being of families during the pivotal early stages of parenting. Participating families receive up to three home visits from a registered nurse who conducts a comprehensive family assessment and provides connections to community resources to promote the health and wellbeing of the infant, caregiver, and whole family. When implemented to fidelity, this voluntary program is offered at no cost to all families who give birth in a participating hospital.
